Introduction:
Dapoxetine, a selective serotonin reuptake inhibitor (SSRI), has been associated with a higher incidence of premature ejaculation (PE) in men with PE. This study compared the efficacy of dapoxetine and the SSRI Cialis in the treatment of PE in men with PE. A randomized, double-blind, placebo-controlled, parallel-group, flexible-dose study comparing dapoxetine and the SSRI Cialis was performed in a total of 96 consecutive men with PE who were diagnosed with PE and who were willing and able to participate. Patients were randomly assigned to receive dapoxetine (2 mg/day) or the placebo (0.4 mg/day). The study patients were followed up every 3 months for an average of 6.3 years. The incidence of adverse events was similar for dapoxetine and the placebo, with a mean of 3.1% and 2.1%, respectively. There were no significant differences between dapoxetine and placebo in terms of the number of adverse events.

The study was designed and conducted in accordance with the principles of the Declaration of Helsinki and was approved by the Research Ethics Committee of the Faculty of Medicine, Medical University of Zagreb (Protocol number: FK2018-0324). Patients were excluded if they were: (i) patients with medical history of premature ejaculation or erectile dysfunction (ED); (ii) patients with a history of premature ejaculation within the last 3 months; (iii) those who had a previous history of PE; and (iv) those who were on any medication within the last 2 weeks before the assessment. The study protocol was reviewed and approved by the study committee and written informed consent was obtained from all patients. Patients were asked to participate voluntarily after a baseline assessment and the following measures were recorded: the patients’ age, sex, PE duration, and PE severity. Results:
In the subgroup of men with PE, dapoxetine and the placebo significantly decreased the time to first ejaculation (t0 = -0.36, P =.02), and the mean of the first 3 to 4 times the mean time to first ejaculation (t0 = -0.48, P =.03) was significantly greater than that of the placebo. In the subgroup of men without PE, the mean time to first ejaculation was 2.9 times greater than the mean time to first ejaculation (t0 = -0.28, P =.01).
Conclusion:
In this subgroup of men with PE, dapoxetine and the placebo significantly improved the time to ejaculation and the time to ejaculation times, but the mean time to first ejaculation was significantly less than that of the placebo. This suggests that dapoxetine may improve PE treatment outcomes in men with PE.
